ALAP Registration

Please use the form below to register your lake for the 2026 sampling season. All lakes, both returning and new, need to register so we can prepare kits and schedule training. Please be sure to complete all the required information, even if you have participated for several years in the past. The Lake Monitor is the person collecting the sample, while the Lake Sponsor is the contact for the invoice; sometimes the Monitor and Sponsor are the same person, but usually they are different. Note that all bottles and filters will be mailed to the monitor. Thank you very much.
